| Registration Form U13-U14 |
This is the registration form to enroll a player into the CESA Select program U13 & u14. |
| Registration Form U15-U18 |
This is the registration form for FALL ONLY to enroll a player into CESA Select program U15-U18. |
| Medical Release Form |
This is the medical release form that gives us both information concerning a CESA Select player's medical history and condition and allows us to get treatment for your child in the case of a medical emergency. |
| Financial Aid Form |
This is the optional financial aid form for CESA Select players whose parents can't afford the fees associated with CESA Select play. Spanish version here. |
| Coaches Evaluation Form |
The coaches evaluation form is used toward the end of the season for each CESA Select player to evaluate his or her coach. This feedback is used to help CESA provide increasingly better levels of service that match and exceed the expectations of our players. |
| Volunteer Disclosure Form |
This is the form that CESA submits to one of its affiliating organizations, in this case the SCYSA, regarding each CESA volunteer (e.g., coach, assistant coach, referee, volunteer, etc.) |
| Combine Registration Form |
This is the form for players to be evaluated by college coaches sponsored by CESA and Adidas |
| CAP Registration Form |
This is the form for players to register for the Spring 2008 CAP Program.Registration for the CAP program will also include entry into the CESA Player Combine. |
