This is the online link for registering a player for CESA, CESA Anderson or CESA Spartanburg U13-18 Travel Team. |

If your child was born in a foreign country (even if US Citizen), please submit the corresponding form (see right) with his/her CESA Registration.
This is a one time only requirement. If you have completed this process with SCYS in the previous seasons you will not need to complete again.

US Soccer Federation (USSF) requires that one of the following forms must be submitted before your child is allowed to play.
- If player moved to live in the US at 11 years of age or younger (Includes US Citizen who was born abroad if they have played soccer in another country), please complete USSF PROOF OF ENTRY PRIOR TO 12 YEARS OF AGE SUBMISSION FORM P12 5-11 and submit along with a copy of a school report card or immunization record issued in the US. Document Must be from before they turned 12 years old.
- If player was born outside of the US but is a US Citizen (includes player with "birth Abroad" birth certificate), regardless of player’s age, please complete and submit USSF FIRST REGISTRATION FORM FR-11.

| Medical Release Form |
This is the medical release form that gives us both information concerning a CESA, CESA Anderson, CESA Spartanburg Select player’s medical history and condition and allows treatment for the player in case of a medical emergency. This form also is required by SCYSA before a player can be registered. |
