This is the registration form to enroll a player into the CESA Recreation program. |
| Financial Aid Form |
This is the optional financial aid form for CESA Recreation players whose parents can't afford the fees associated with CESA Recreation play. The Spanish version is here. |

This is the worksheet for the team parent (sometimes called the team manager). |

This is the worksheet for the team parent (sometimes called the team manager) specifically for the U6 2-Day teams. |
| Volunteer Disclosure Form |
This is the form that CESA submits to one of its affiliating organizations, in this case the SCYSA, regarding each CESA volunteer (e.g., coach, assistant coach, referee, volunteer, etc.) |
| GHS Medical Release Form |
This form is not online. Please obtain one from your team manager, complete and return to the team manager who will then give it to the GHS athletic trainer to have on file in case of accidents. |

This form is for coaches that need to let the CESA office know about an incident during a recreation game. |
