This is the registration form to enroll a player into the CESA Academy program ages 8-11 as of July 31, 2009.

| Medical Release Form |
This is the medical release form that gives us both information concerning a CESA Select player's medical history and condition and allows us to get treatment for your child in the case of a medical emergency. |
St. Francis Consent & Authorization Form

This is the form we need on each player in order to receive medical attention in case of an injury.

This is the optional financial aid form for CESA Academy players whose parents can't afford the fees associated with CESA Academy play. Spanish version here. |

| Coaches Evaluation Form |
The coaches evaluation form is used toward the end of the season for each CESA Select player to evaluate his or her coach. This feedback is used to help CESA provide increasingly better levels of service that match and exceed the expectations of our players. |

This is the Lloyds individual player order form. |
| Team Uniform Order Form |
This is the Lloyds form that the team manager should use to turn in orders all together as a team. |
Training Shirts Order Form

This is the form to use for individual players to order the Vapor micros set of red, white & grey.
